Community Health Needs Assessments and Implementation Plans

CHRISTUS Health works in close collaboration with local nonprofits, public health agencies, educational institutions, and other community organizations to conduct Community Health Needs Assessments (CHNAs). These assessments help us identify the most pressing health needs in the communities we serve, with a special focus on those who are poor and vulnerable.

Our approach is both community-centered and data-informed. We engage with community members through surveys, interviews, and focus groups to better understand local strengths, challenges, and opportunities for health improvement.

In this current cycle, we emphasized understanding health needs across life stages, from early childhood through older adulthood. This perspective allows us to design strategies that are responsive to the evolving needs people face throughout their lives and to address health disparities in a more targeted and meaningful way.

Based on community input and data analysis, we identified key health priorities. These are reflected in our Community Health Implementation Plans (CHIPs), which outline coordinated strategies to address both immediate needs and the underlying factors that influence health and well-being.
